After completion please email a copy to Mrs. Greek or print it out and turn it in at our welcome table on Fridays!

Release of Liability
Form
**Make sure to fill out the Release of Liability Form before attending any events!**
Medical Release forms are required of every student attending an ASB event. You only have to fill it out once for the entire year. You can either print it out and bring it to the next event you attend, or email it to Mrs. Greek.
