Release of Liability
Form
**Make sure to fill out the Release of Liability Form before attending any events!**
Medical Release forms are required of every student attending an ASB event. You only have to fill it out once for the entire year. You can either print it out and bring it to the next event you attend, or email it to Mrs. Greek.
2025-2026 ASB Application
Applications are now open for ASB! If you're interested in applying, please print out the document below and either email a copy to: karengreek2@gmail.com OR turn in your copy at the welcome table on Fridays. All applications are due by April 25th!
Prom 2025 Dress Code form
Please print out and sign before the event. All students and guests must fill this out. Dress Code Forms will be turned in at the check in table upon arrival at the Prom venue. See you there!
